LSTC - Grafton, ND - Licensed Practical Nurse Work Area: Maplewood/Cedar Grove Status: Salaried benefitted position Work shift: 12-14 hour shifts, every 4th weekend and every 3rd holiday. As an LPN, you will provide direct nursing service for people served with intellectual disabilities ranging from medically complex to dual diagnosed. Assuring health care needs/standards are met as well as working to provide training on promoting independence. This position will oversee and train medication technicians, provide acute assessments as well as ongoing comprehensive assessments for people served. Provide education to employees, people served, teams and family/guardians, and coordinate with a team of professionals to achieve best possible health outcomes. The primary role of the Nurse Case Manager is to coordinate care and services for injured workers who are covered by North Dakota Workforce Safety & Insurance. Duties include:Manage a caseload average of 20 to 30 cases depending on severity. Respond to catastrophic or complex injuries as requested by the claims adjuster. Initiate contact with the injured worker, treating physician, and care providers to thoroughly report the extent of injuries to the claims adjuster.Conduct daytime travel to surrounding areas for on-site visits to physician offices, physical therapy programs, hospitals, etc., to coordinate medical services for the injured worker, and assist in coordinating medically related services.Serve as a medical resource to the claims adjuster, legal department, and Workforce Safety & Insurance employees and provide monthly reports to the claims department on each case being managed to document case progress.Develop and file case management plan, correspondence, and documentation and reviews and/or revisions as needed for each case.Provide professional-level analysis of medical-related information in client treatment, therapy medications, cost containment measures, etc.Must have a valid driver's license and be able to drive and maintain a State vehicle.Communicates clearly, effectively, and professionally to those internal and external to WSI.

Minimum Qualifications

The individual will work as a team member in an infirmary/clinic setting and within the Medical and Psychiatric Service Program. The position requires a holistic approach to health care service with emphasis on health maintenance through patient education and positive self-initiative. Specific duties include but are not limited to: provision of health care nursing to the resident population; dispensing medication; emergency nursing care; participation in behavioral management procedures and infection control; thorough documentation of patient care and medical records; and individual, group and wellness education. Minimum Qualifications

Must be licensed as a Registered Nurse or LPN by the ND Board of Nursing. Requires interpersonal skills and communication skills necessary to perform job duties and must have ability to work with an inmate population on a daily basis. Strong clinical skills required. Psychiatric, medical surgical, and critical care background preferred, but not necessary. Individual must be flexible and innovative in carrying out assigned responsibilities.
