Prevailing Wage Request Forms
The prevailing wage request form is a form to collect pertinent data related to an employer's request for a prevailing wage for a particular occupation under a particular section of the immigration law (H-1B or H-2B or Permanent, etc.) Federal law says the employer requests must be in writing and that certain items must be included in the request: job title, description of job duties, education, training and experience requirements, name and address of the employer, a contact person and telephone number, as well as the city and state where the alien will be working along with any other items required by the state employment security agency to process or track the case. The prevailing wage form exists to capture all of this data and serves as a record for the state employment security agency and the employer as to what wage was assigned and when.
